Doctor Appointment only $75

To start the process for certification fill out the following information and you will be directed to an online payment system to arrange a consultation with a doctor.

Gather any medical records prior to your doctor visit and a $75 consultation fee for Current medical marijuana patients and $100 for NEW patients.  Should the doctor not certify you the use of medical marjuana your application fee will be refunded less a $25 administration fee.
